What causes Errors 1095C-010-01, 1095C-039-01, 1095B-010-01, 1095B-055-01?
This typically happens when the last name and SSN do not match what the IRS has on file.

This can happen for the following reasons:

  • Maiden name used when married name is registered with the SSA (or vice-versa)
  • Hispanic last name used where 2 last names are common.
  • Covered individuals can cause this error.
  • Employer EIN mismatches on the 1094-C, Part I
  • Aggregate ALE EIN mismatches on the 1094-C, Part IV.

  • Error 1095B-010-01 applies to Form 1095-B Responsible Individuals.
  • Error 1095C-010-01 applies to Form 1095-C Employees.
  • Error 1095B-055-01 applies to Form 1095-B Covered Individuals.
  • Error 1095C-039-01 applies to Form 1095-C Covered Individuals.
  • How do I fix Errors 1095C-010-01, 1095C-039-01, 1095B-010-01, 1095B-055-01?

    Here are a few suggestions:

  • Verify the data you are filing matches a signed W9 or I9.
  • Contact your employees to get the correct information and document this thoroughly. According to the IRS:

    Due to privacy/disclosure issues, we can only inform you that the name and social security number do not match our database. Ask your client to double check the information and if he still believes there is no error, have his employees contact IRS customer service who can direct them properly. You or your client won't be able to act on the taxpayer's behalf without a proper POA so that is why we recommend that the employees in question call IRS to confirm what name and SSN is on file for them. The customer service number is 1-800-829-1040, or they can contact their local IRS office. There are also specific numbers per state on IRS.gov they can use if they wish. Have them visit an IRS Taxpayer Assistant Center for those numbers. There are no penalties for 'good faith' efforts.

  • Try alternate last names. Remember, only the first 4 letters of the last name are looked at for determining an IRS match, so if a last name like Smith-Jones doesn't match, you may want to try Jones-Smith.
  • Remove the SSN for covered individuals on erred records, and use the date of birth instead.
  • Correct your employer EIN or name (if the error happens on a 1094-C).
  • Update the Part IV Aggregate ALE section if necessary.

    You need to install the latest service pack.

  • From the Help menu, select About Account Ability (Check for Updates).
  • Click Check for Updates
  • Follow the instructions displayed.

  • This will fix the problem.

    Why am I getting the error "Self-Insured coverage conflict" when trying to create my 1095-C transmittal?
    IRS business rules require that you check the box Employer provided self-insured coverage if, and only if, there is at least one covered individual. If the employee and all of his/her dependents opted out of the self-insured plan then you must uncheck the Employer provided self-insured coverage box.

    Consequently, a "Self-insured coverage conflict" error will occur if either,

  • The employee and his/her dependents opted out, but the Employer provided self-insured coverage box is checked, or
  • The employee and his/her dependents opted in, but the Employer provided self-insured coverage box is not checked.

  • FYI: The 1095-C in error will be displayed when Account Ability stops on this error.
    TIP: Adjust Self-Insured Indicators on the Tools menu of Form 1095-C will correct this error on all 1095-C forms.
    Why was my submission rejected with an AIRMF3001 error?
    A bug in the IRS e-Services system inadvertenty moved many software ID numbers into Resubmission mode. Unfortunately, Account Ability's software ID was affected by this bug.

    IRS has informed us that they are working on the solution and suggests trying again and keeping a diary of each submission's Receipt ID.

    How to Proceed:

  • Open Manifest Management and Testing on the Module menu.
  • Click once (or right-click) on the transmittal that was rejected.
  • Click the Notes button on the button bar (or from the popup menu).
  • Enter the Receipt ID, Date, and error code AIRMF3001 in the Note register.
  • In 7-10 days, double click the transmittal and create a new manifest.
  • Do not mark the Type of Records as a Replacement.
  • Try submitting again.

  • If you should receive a late filing penalty notice from IRS in the future, contact IDMS at 631-249-7744 for instructions on how to abate the penalty.
    IRS has informed me that I only need to correct Form 1094-C. How do I do this?
  • From the File menu of Form 1095-C, select Transmittal 1094-C Options
  • This will display 1094-C pages I, II, III and IV
  • Make the necessary changes to each page
  • Check the CORRECTED box on page I
  • Click OK
  • From the File menu, select Electronic Reporting
  • When the Form 1094-C options are displayed, verify your changes and click OK
  • You will now be prompted for the RECEIPT ID of the transmittal to be corrected
  • Click OK
  • Your electronic transmittal will be created and ready for manifesting
  • Now proceed as usual to manifest and upload to IRS AIR
  • From the File menu of Form 1095-C, select Transmittal 1094-C Options
  • Uncheck the CORRECTED box on page I
  • Click OK

  • Check your results with IRS AIR in 24 hours.
    How do I correct error AIRMF3014 - File is a Duplicate?
    If you received error AIRMF3014 when submitting a Replacement file, you will need to slightly alter the 1095 source data and create a new transmittal. The Replacement manifest for this transmittal will be assigned a new ChecksumAugmentionNum, which should eliminate the error.

    The following suggestions for altering 1095 source data need only be done for a couple of records, not the entire file:

    Change STREET to ST, LANE to LN, CIRCLE to CIR in the street address.
    Change ROOM to RM, SUITE to STE, FLOOR to FLR in the street address.

    Enter a ZIP CODE Extension - You can look up ZIP CODE Extensions by double-clicking the ZIP CODE.
    Can I use my IRS FIRE TCC to file Forms 1094, 1095?
    No. Your IRS FIRE TCC can only be used to file information returns 1098, 1099, 3921, 3922, 5498, and W-2G with the IRS FIRE System. An IRS AIR TCC is required to file information returns 1094-B, 1095-B, 1094-C and/or 1095-C. See the Account Ability Help menu for instructions on how to apply for an ACA TCC ("Application for ACA TCC Instructions").

    Why did I receive an AIRMF3005 error when uploading my Production files?
    Your TCC is probably still in TEST mode. In order to use the Production System, your TCC must be moved to PRODUCTION. Contact IRS at 866-937-4130 and request that your TCC be moved to Production. Once in Production, try uploading your manifest and form files again.
    How can I check to see if my TCC has been moved into production?
    You need to logon to your e-Services Account.

    Production files should be uploaded using the AIR UI Channel Login - Production link.
    You probably used the AIR UI Channel Login - AATS (Testing) link.
    After logging into the IRS website, the website hangs on "Select Organization". What I should do?
    There may be a problem with your TCC application with IRS.

    You need to logon to your e-Services Account.

    Click on the link:
